Patient Forms

NEW PATIENTS: Your privacy and safeguarding your personal information is of utmost importance to us. In order to securely provide your information, please contact our staff at (678)827-9157. Our staff will collect your email address and send you a secure message using Hushmail Secure Email with a secure link to forms to collect your information. The forms include contact information, insurance details, medical history, medication and allergy information, HIPAA release form and our Financial Policy form. You can complete them all online and also send attachments (insurance info, driver’s license, etc) securely. This process will save time for you and us while providing industry leading secure communication. Once we have received and processed your new patient forms, a staff member will contact you via phone to schedule your first appointment with Dr. Shama!
